Supporting Self Management

Welcome to Supporting Self Management a learning resource to help healthcare workers develop knowledge and understanding to support self management.

Supporting people to self manage is a key part of the healthcare response to long term conditions. In your daily practice this means that you need to feel comfortable using a range of tools and techniques which support self management and a willingness to work with people as equal partners in their care.

The resource is divided into four main themes. These are intended to help you feel more able and confident to incorporate support for self management within your daily practice.  There is no specific order in which to work through them. Click on each to find more information, activities and resources.

This resource has been designed primarily for nurses and allied health professionals, however any healthcare staff member would benefit from using it.

A good place to start each theme is the knowledge and skills area where there is a quiz to prompt you to think about your current practice.
