Significant Event Analysis: for pharmacy staff

Submitting analysis of an event that has been significant to you and which has made you reflect on your practice will help with your CPD.

What is Significant Event Analysis (SEA)?

Significant Event Analysis is the analysis of an event that has been significant to you to make you reflect and consider changing practice.It needs to be an 'event' that you have been involved in e.g. an error, a change project, poor communication etc... It is not a way of recording your CPD and so by 'event' we do not mean an educational event that you have attended.

We have progressed and are now able to offer information about the Enhanced Significant Event Analysis.

Enhanced Significant Event Analysis

Enhanced SEA is a NES innovation (funded by the Health Foundation 2012 SHINE programme) which aims to guide health care teams to apply human factors thinking when performing a significant event analysis, particularly where the event has had an emotional impact on staff involved.


Taking this approach will help individual teams to openly, honestly and objectively analyse patient safety incidents, particularly more difficult or sensitive safety cases, by ‘depersonalising’ the incident and searching for deeper, systems-based reasons for why the significant event happened.


In this way, a more constructive approach to learning can take place and more meaningful improvement can be implemented to minimise the risks of the event happening again.

How do I submit a Significant Event Analysis (SEA)?

Submission comprises completion of the SEA template and then e- mailing it to pharmacy@nes.scot.nhs.uk

On receipt of your submission you will receive a unique reference number that should be used in any communication. If you don't receive a unique reference number within 3 days of submtting your SE for analysis please telephone 0141 223 1600 and enquire as to the status of your submission.

Please note that it can take up to 4 weeks for a SEA to be reviewed and the feedback processed.

What type of events are submitted for peer review?

The events submitted for peer review vary. They may be about a:

  • clinical event
  • communication issue
  • management scenario
  • 'near miss'/medication error

The key thing is that the event was significant to you, has made you reflect and possibly alter your practice. Most importantly how you plan by undertaking the analysis to prevent such an event happening again. 

Examples of SEAs

 

Wish to become an reviewer for SEA?

  • What we need is an abbreviated CV and a committment from you to complete the training
  • A committment to review ~ 1 SEA every 4-6 weeks within a 2 week window.

The training comprises of:

  • Completion of the enhancedSEA e learning module (see patient safety web pages)
  • A review of 3 SEA (e mailed to you after your CV has been processed)
  • Participation in a peer discussion about the SEAs that you have reviewed (organised by NES)

On completion of the training you will review your first 2 SEAs with a more experienced SEA reviewer.
